IMPORTANT: Assure you choose the form for the correct aid year. Before submitting all documents, print student name and CSUMB I.D. on the top of each requested item. Please refer to the form for information on where/how to submit the document.

Click on to OASIS
Click on Self Service>Student Center
Missing documents are listed on the upper right side under "To Do" List
For the 2019-2020 school year, the Verification Forms are PDF fillable. In order to save your Verification form to your desktop, we suggest that you first download the PDF form to your computer. Then, you can either type information directly into each field, or copy or paste text. You can save your data and re-open the file later to modify or enter additional information. Once done, please either mail or bring documents into the Financial Aid Office.
